Upon submitting the registration form with The Dance Warehouse, you agree to the following:

 "I hereby enroll (student's name) and agree to the following terms and conditions:

I agree to follow all studio rules and guidelines including, but not limited to, payment policies, dress codes, class attendance as well as other studio rules and guidelines posted in the studio or on the website. I understand that all tuition payments are nonrefundable and due by the 10th of each month. If I do not pay the tuition by the 10th of the month, I agree to pay the standard late fee. I understand that all costume payments, competition and workshop fees are due on or before the posted date, and refusal to pay on time will also result in a late fee. I understand that there will be a service charge on all returned checks. I also understand that in order to withdraw from the program, I must submit my withdrawal in writing, or I am responsible for all tuition, costume payments and workshop and competiton fees. I understand that by participating in a dance class, I am exposing myself or my child to risk of injury. I accept this risk, and agree that The Dance Warehouse and its staff will not be held responsible should such injury occur."
